Bad news I'm afraid - looks like I've lost that booklet. However, I did remember this post, which pretty much says everything the booklet does.

I've just been looking for the booklet - I can't find it right now, but I'll keep looking. Would it be too much of a problem if I get it to you by Friday night? I'll try and do it beforehand, but I'm just warning you that it could take longer than I hoped.
