errm i'll try to help but I might confuse you even more. 
If i were you, I would actually keep maths and drop physics for history. This is what I feel now, after taking three sciences. I actually feel like I should have taken another subject instead of physics. History would be great to do in its place. But Maths is good to keep because its a difficult subject and it shows perseverance. Physics is verrrry difficult and it overlaps with maths anyway. Plus its not needed for medicine and I feel taking an unusual subject would actually be beneficial. 
Doing all three sciences is pretty hard, because its a lot of theory and you end up having a lot more to learn for exams. Whereas with essay writing subjects like History, you have part coursework and essay based questions, so thats based more on your analytical skills, whereas with the sciences you actually have to understand everything and learn it for the exams.
Every A-levels subject is a LOT of work, nothing is easy to get an A*/A in. The leap from GCSE to AS in any subject is massive, but I guess it depends on you as a person. If you have good essay writing skills and analytic skills then i would definitely encourage you to do history but I will warn you now, whatever subjects you choose, you will have to work ten times harder than you are now. It is a lot harder, not trying to scare you.

Hey, I did maths, further maths, biology, chemistry and history at AS and then dropped history at a level. You need chemistry and I would recommend biology and then either maths or physics. Then choose another subject which you enjoy. Further maths doesn't really add that much to your application but takes a hell of a lot of work so I would probably recommend not doing it but it's up to you

They're both fine in terms of getting in, Med Schools won't mind which you do 
You need to be confident of getting an A, obviously, but other than that, go for the one you enjoy more!

I'm a graduate applicant so my A-level choices are not important. If you want to go into Medicine straight after school then you need to do Chemistry A-level and 2 others plus Biology AS minimum. Ideally you should do Bio, Chem and maths/physics and an AS. I would suggest a contrasting AS such as philosophy to offer an ethics and humanist perspective.
