Designing a consent form and a debriefing form...

I have already posted a thread relating to an investigation/report which I have now managed to complete, however with regard to my project proposal form I'm unsure as to what both a consent form and debriefing form should include exactly?
It's just something that I don't recall studying specifically in class, I have searched online for examples of both. But they're mainly samples from more detailed ones used by universities, which aren't necessary for what I need to produce.

Also in terms of the consent form, I have had to use prior general consent rather than informed consent. So I can't reveal the true purpose/aim of my investigation. Does this mean completely disregarding the purpose on the form, or making a false purpose? :s-smilie:

So just some more simple pointers would be much appreciated if possible. So thank you anyway for reading this etc.
