Make a timetable and stick to it. Give yourself regular breaks. Drink lots of water. Don't stay up all night or cram. Do exercise.

When you revise, revise away from your computer (if you can). Don't just sit in front of it and turn the screen off, that doesn't work. If you're using a laptop, leave it somewhere else. Maybe run the battery in to the ground and put it somewhere on one side of the house, then put the charger on the other side of the house. Do past exams, any question you get wrong, go over why and learn the subject of that question so well that you don't even have to dominate it, it will just see you come in and bend itself over the table so you don't have to. It might be worth, every time you feel like stopping and doing something pointless, asking yourself "Is watching TV/playing Skyrim really worth failing my GCSEs?" Remember, though, that GCSEs aren't the be-all and end-all if you're going on to A-levels. Nobody cares about GCSEs once you have a higher qualification. That being said, they're still pretty important. Especially if you don't get a higher qualification.

Have regular breaks, drink lots of water, take notes on what you're reading, do practice tests/exams. When I had to revise for a psych A-level, I sat down at the table with only my book, relevant textbooks, a cup of tea (recommended, also a glass of water), a plan, some blank cue cards, and I spent the next 3 days taking notes. I later got a big, fat U on that exam because I drew something on a question I couldn't answer (didn't make it that far with the revision, started too late) once I'd finished and didn't cross it out. They dubbed my beautiful illustration as "offensive" and gave me nothing for that exam. What a bummer/pointless waste of both my time and theirs. /rant

tl;dr Remove all distractions, set yourself a target and give yourself a revision plan (make it fun, associate each topic/subject with a room). Drink water. Have breaks every hour. Do past exams.

EDIT: When you have a break, do something active if possible.
