    DSA Renewal
    I have a diagnosis of Aspergers Syndrome and I have mental health issues. My last DSA application was 4 years ago and I have been advised to fill in another application form. I am currently on a leave of absence due to health grounds.

    Do I fill in the short form?
    Do I include my previous application?

    I will be getting a Doctor's letter but my educational psychology report is 4 years old.

    Its best to ask your disability adviser at your university which form you need to use. Provided your educational psychologist report was completed after you were 16 years old i.e during college or after, then the fact it is 4 years old shouldn't be a problem.
