Evidence for Dyspraxia
Not sure which forum to post this on. Anyways I have dyspraxia, which severely affects my writing. and I received notification from my university choices that I need to give them evidence that I have got it only problem is I have no documentation at home at all which proves it; since I was diagnosed at school does anyone know of a way to get such evidence. It will be much appreciated.

You need to see an ed pysch really for proper testing. Your uni or place of education should actually fund that for you. Google and look up the university's disability service, either email or give them a phone.
For a formal diagnosis you must have, at some time, seen a school doctor or a paedatrician or similar. I have a box of reports for my dyspraxia and saw a school doctor, a paediatrician, my own GP, a pysiotherapist, occupational therapist, educational pyschologist.. Normally these people will all write reports which should be held on your NHS records. Sometimes paper copies get lost but if you have had a formal diagnosis then there will be something on your notes relating to it.
Normally you will have to retake any assessments with an appropriate person (ed pysch) which the uni should accept as proof of your disability. Sometimes though they can be fussy and might like you to be assessed by someone of their choosing (especially if they are paying) hence why it's best to speak to them first. -

Think the initial test was done at school when they said there was a strong chance I was dyspraxic - but the formal diagnosis was made by a paediatrician at children's hospital.. Might be a different procedure for adults though.

I was also assessed at a hospital for Dyspraxia, my teacher when we were living in Saudi Arabia and my mum both saw the traits in me and then I was assessed and diagnosed at a hospital in England.
If you were diagnosed at school they should have a record of it, and maybe an educational statement. -

Re: Evidence for DyspraxiaAre you in school at the moment? If so, you will be able to receive a free assessment. Your files should also be stored for some time and you'll be able to get it by simply phoning them and asking.(Original post by omegad)
