So my dad is getting a 'Final wage-slip from 2014-2015' but my mum is a carer for my sister as she is disabled but we don't have any evidence/ know what evidence to send off with her form. Do we have to send something off or can we just send my dad's wage-slip off?

Does your mum or your sister receive any taxable benefits (both ESA and Carers Allowance are taxable even though most recipients never earn enough to pay any tax)? If so they would have received a P60U form from the DWP last April which would need to be provided as evidence.
