Abitur - UCAS form

I'm completing my German Abitur next year and I want to apply for uni in 2010.
But the problem is I don't know how to fill in my grades (12.1, 12.2) in the UCAS qualification section, if I have to. Do I create separate modules for every semester? Do I have to put in every grade since year 12? Are the Zentrale Klassenarbeiten relevant?

That's what I did (it says so on the British council website as well)
e.g. English LK (intensive course) 12.1 -> 14 of 15 points, or something along those lines. Have a look at this: http://www.britishcouncil.de/d/education/ucas_online.htm
