UCAS Reference help

I've just received my reference from my music teacher and it asks for:

'Post/occupation/relationship'
'Organistation'

He's a retired industrial chemist, how exactly do I go about filling this out?

I have the same problem. My reference is from my ex private English tutor. I don't even know how you can post the reference if you apply independently , do you?

In 'Post/occupation/relationship', the last is in this case the important one. So, enter 'music teacher'. For organisation, enter the organisation through which he teaches you (for example your school) if that's the case. Otherwise, I believe you can leave it empty, but I'm not sure about that.
