Tell your exams officer/head invigilator before the exam starts. They will make a note and contact the exam board. You should get "special consideration",only a small increase in marks.
Yep must be done the day of the exam. Also if it's a close relative your school should contact any unis you've applied to to tell them about your circumstances (usually by letter) the unis may then be more lenient if your grades are affected.

For special consideration (a few extra %) a form has to be sent to the exam board within 7 days of the exam taking place. You need to talk to your school's examination officer for this.

Grandad should easily be close enough to get special consideration. And may (depending on your school) be enough for them to send an extenuating circumstances letter to your Unis. This would normally be your Head teacher who writes the letter.
