What should the format for a "requesting feedback" letter look like?

I was rejected by LSE for L101 Economics, and I want to get feedback on why this was the case. What should the format of my e-mail be, and to whom should I send it to?

You should send it to the admissions department, and it doesn't have to be too formal. Just tell them your offer wasn't accepted and want to ask why.
