No it's by no means a certainty the way your teacher implies. I've heard of cases where the offer has been higher to make sure the person continues working hard. It's far more likely that you'll be given the typical offer though.

Depends entirely on institute and subject. Typical offer is just that - the typical, average offer. It can be higher or lower depending on a set of circumstances which is individual to that specific university and department. For example, it can be lower if they really want you to come, or higher if they want you to continue working hard. Most people receive the typical offer, though. That's why it's the typical offer! :p:

Unis are not interested in being your 2nd choice, so they may give you a higher offer if they like you. People pick (incorrectly) in grade order, so a higher offer insures that uni comes first.
