I think its pretty likely that they do, just like alot of girls think of other guys when they masturbate. And while I recognise that it would be unreasonable to expect him not to think of other girls while having a tug, it makes me a bit insecure to know that he'd rather that I looked like whatever celebrity/pornstar/actual person he is thinking about, and that he'd rather be ****ing them :/ its a bit sad really.

Your boyfriend wouldn't be dating you if he wished you looked like someone else, he would be dating them instead. The same thing applies to porn. Girls seem to have this crazy idea that guys think porn stars are good looking.
