HC1 Application

    Am I eligible to apply for a HC1? I'm 21, at university, with no other income other than student loans.

    Need a tooth ache sorted but don't have the money to pay for it :/

    You'll also have to fill in a HC2 form, which is basically for prescriptions and dental work.

    Yes, you can. Ask your pharmacy for a HC2 form. It will cover prescription costs, optician check-ups and dentist appointments too. The certificate last a year and its best to keep it with you when attending such appointments.

    Your dentist surgery may even have the forms available.

    Won't it be the HC1 form as this is the form you use to apply for help with health costs. HC2 is a certificate that gives you full help with health costs and you get it through the HC1 form.

    Yes fill in a HC1 form, you get a H2 certificate and if you have had to pay for any work done or check ups then there is another form you can fill in to get your money back once you get your H2 certificate.
