How do I stop being a hypochondriac? I keep worrying about my health all the time!

How do I stop being a hypochondriac? I keep worrying about my health all the time!
I don't know what's wrong with me, I have terrible hypochondria. I have anxiety and mild panic attacks too, which I've been suffering from for about 2 years, but every little pain or breathing issue, I worry about a heart attack or it's something to do with my lungs. When I have anxiety and panic attacks, I get headaches and slight dizziness, then I worry it's a tumour or something more serious. I am making myself more anxious because I'm worrying about all this. I can't believe I'm thinking like this, especially as I'm 23 years old!! I just want to be healthy, but at the moment I'm worrying about my health.
How do I get over this?
