Help needed.. what do I put for pre-acceptance health questionnaire?
Hellooo there all.
I've been accepted for nursing and I'm filling out the health questionnaire thing..
Thing I'm scared about is these questions: Have you ever attempted suicide/drug overdose/self harm and have you ever suffered from anxieties, addictions, bipolar disorder, schizophrenia etc? Give dates and explanations.
I've had depression and anxieties in the past (I mean who hasn't?) and I've spoken to my doc and he said that I HAVE to include all of this
I'm so worried that I am going to be rejected after everything just because of the mental health section? I've not been diagnosed with anything even though I was reviewed and I've not been having treatment for years. I can't be the only person applying that has suffered with depression??? I'm completely sane!!! -

I would declare it. The reason they ask is to see if they need to make any changes for you, ie if you need extra support etc.
Mental health problems are considered a disability so you are protected by a lot of legislation.
I think all they will do is ask your GP how you are, and if you are fit to start the course. Nothing to worry about. -

Its better to declare it than not and to be honest, all they want to know is what happened, how long ago or if its still a problem then how it affects you, how many days off you have because of it. I was worried about this part as well but not due to mental health aspect with me it was physical and those are pretty much the questions i got from occupational health

And there is a couple of people with depression in my cohort at the moment so you should be fine

I have no choice but to declare or my GP won't sign it.
