There are some flaws in the survey. Some are spelling, which aren't really very important, but you need to include more age brackets such as 16 and younger, because while you said all students are allowed to fill out the survey, there is no age bracket for them.

I'm also not sure if you need to specify that the information received will be confidential, because it isn't personally-identifiable.

Anyway, once you've fixed the age thing then I'll fill it out.

Agree that the flaws mentioned by ThatPerson need to be fixed. Also, the question about what course one is studying should be changed as it's currently relevant only to university students (I answered by putting my A levels).
