How would you say "he acted as if he only had a few days to live" ?

Er handelte/ verhielt sich als wenn er nur [noch] ein paar Tage zu leben hatte.

Could you use "als ob" and then the subjunctive as well?

Yes, but I'd suggest to add a "so", i.e. Er handelte so als ob er nur noch ein paar Tage zu Leben hatte. You don't need to use the subjunctive here.
