Oxbridge Clinical Transfer

I am thinking about applying to Cambridge but I'm a little concerned about the need to transfer to a different university for clinical studies. The MOCAG application form asks for a description of extracurricular activities and work/voluntary experience - could anyone indicate how significant they are to an application for UCL and Kings?

I understand there is fierce competition to get a place at both of these unis -How much time and effort should be put in to this in order to be successful?
