Why do changes in temperature and pressure bring about changes in phase?

Ok so you have an equilibrium between the two phases. In one direction the phase change is endothermic and the other is exothermic. As for pressure, you are simply applying Le Chatelier's principle to an equilibrium between 2 or more phases. That'll have to do unless you want lots and lots of maths and stat mech.
