The exact mechanism will depend on the cause of the end-organ (aka target organ) damage.

Take hypertension as an example, chronic increased load and work on the heart leads to hypertrophy of the muscle, and ineffiency, and there's your CVD. There are other ways as well, and risk factors are going to overlap in an actual person, think hypertesnion, think hyperlipidaemia, think atherosclerosis which will also lead to CVD
