worried about not meeting my conditions

I have now finished my access course and I am just waiting for my final few assignments to be marked. I needed 36 credits at merit or Distinction. I so far have 27 Distinctions and 6 merits, so I am pretty sure I will be fine by quite a way. However, my uni place is now hanging on my achieving a C at GCSE maths. I am worrying myself sick as Maths is very difficult for me. I do hold a level 2 maths qual which I did alongside my access course but I still need a C in maths GCSE. I question is do you think I stand a chance of still getting into uni if I get a D again or I am looking at a failure here? I cannot re-apply next year as they want GCSE science which I do not have.

Only the uni will be able to confirm, but Grade C Maths and English GCSEs (or equivalent) are usually a basic requirement for uni entry. They ensure a level of numeracy and literacy which mean that you won't struggle with a degree course. That's the idea behind it, anyway.
