After not receiving a decision on Thursday morning i presumed that both my firm and insurance were still considering my application. The following morning both universities informed me that they hadn't received my results. I then had to send the results myself. Who lays with the fault for this - the exam board or Ucas?

The following morning both universities informed me that they hadn't received my results. I then had to send the results myself. Who lays with the fault for this - the exam board or Ucas?
The most likely reason is that you never put your UCI (unique candidate identifier - looks like 111110002222A and is used to tie all your exam results to single identity) on your UCAS application. Without it they won't be able to find your results.
