Should I mention that I am being monitored by SEN on my UCAS?

When I was younger, I used to have a full statement for Special needs due to communication difficulties. The full statement got taken off when I was 11 or so (I guess) but I am still being monitored by the Special Needs Programme at my school. Should I mention this on my UCAS or not?

Where would you mention it? If you mean on your personal statement, I wouldn't mention it unless you think they need to know.

If it's actually an issue, mention it. If not, don't. I don't think you're obliged to declare it.
SEN ends at the end of high school.
