what does it mean when some say hey how are you

what do they even want? What are you even supposed to reply?

Because it's part of social construct and unless they know you well, they're only expecting something like good/fine/ok.
they don't actually want to know how you are, it's just a greeting. The response is "fine thanks" , not your whole life story. It's a greeting, they don't actually care.
Wouldn't it be easier to just reply with how you are?
They're basically telling you that they want you and your entire family murdered. By engaging in conversation with them, you are pretty much giving them the go ahead to commit these acts.
