You are supposed to include all grades including unqualified's. I've heard of people choosing to not have their subjects aggregated and not including them but any admissions team will see right through you if you only put Maths down.

If you choose to include all of your subjects make sure you justify retaking and talk about your huge improvement in a positive way. Don't say that you didn't try or didn't care before. My friend managed to turn his previous failings into a pretty strong advantage as he talked about having taken subjects relating to maths and realizing that maths was what he truly wanted to study.

I don't recall putting down my U grade, no (I'm going into 2nd year now). But to be on the safe side I would phone UCAS, they'll be able to tell you if they've changed anything. From what I remember it was if you didn't get a certificate in the subject (in other words you got a U) then you don't have to write it down in the form.

With regards to the E grade you would need to put it down, unfortunately.
