Scariest/most memorable Game Over screens/deaths

I remember this game Pandemonium, where if your jester character died they had an actual heart attack, the screen went black and the music jarred from the stage theme to the dramatic Game Over sound. **** my pants when I was 9 :/

It's good, but would I say it's the GOAT? Oooh, that's a tough one. Resi 1 -3 were magnificent and 4 was my favourite game of its generation, but 5 really pissed me off! :lol:
