Survey about the nhs

Hi, I'm studying a healthcare access course and am required to write a research assignment. I have devised a survey here and would be very grateful if you could spare a few mins to answer the questions, there's only 10. The results are completely confidential and will only be used to denote how the public feel about the subject.

Hey, I've completed yours. Here is mine:

It's for smokers (even if it's jut 1 a year), and ex-smokers who can fill it out thinking back to when they used to smoke! :smile:
