How A pH Meter Works ? URGENT!

Anybody know how one works? I am writting up my biology coursework where we used one which was a like a probe on its own in the experimental procedure and have to explain how one works, I think it was called "checker" by hanna. It would also be useful if someone knew where I could get a diagram of how one works aswell. Any help would be awesome, rep on offer!

I think all you have to say is that you get an output voltage which is proportional to the pH of the solution

pH is the concentration of H+ ions. Defined as -Log [H+] Where [H+] is in mole/dm3.
