Is it possible to apply for a different course in one uni and the other 3 with the same course?

Yes but you would struggle to cater your personal statement to both courses (and that's generally a bad idea to do).

Thats why l chose them strategically because l want to do a courses in health...is it still possible?

when you say health do you mean health professions or do you mean like public and global health which will have similarities?
Depends. If they are really similar - say History/Modern History/Military History - you can structure a PS around all three. If they are wildly different say - Optometry/Biomedical Sciences/Physiology - you would struggle.
