GCSEs and University application

I know A levels are more important than GCSEs, but how valuable are GCSE results in terms of getting into a top uni?

If I wanted to study a competitive course e.g.medicine, chemistry, dentistry etc. and I got 3A*s at A levels, and someone else got exactly the same, would universities then look back at GCSE grades and pick the student with the better grades?

Also, do universities only ask you how many A*s you got (separately), or how many A*s AND As you got (collectively)?
