can I swap my choices with ucas track if I'm an early applicant?
I've applied for medicine. But I want to swap my fifth choice. Can I do that? Can I do it after the October 15th deadline if it's not a medicine choice?

Yes and Yes
You have 7 days after your welcome email to do it via track - and another 7 days after that when you can do it over the phone.
