the one who needs it most desperately for medical reasons? (unless ones higher on the transplant list?)
The transplant list is ranked in order of medical priority, in theory.

if it is hard to decide, tell them that. its not that you are meant to make a life or death desicion in 2 minutes, its how you approach the question. refelct on both sides i.e. for and against, make sure you show them that you know that medicine is about team work so ask your colleagues and if you still cant arrive at a decision at that moment in time tell them so! they didnt eat me up, so........

you can't not give it to the ex-alcoholic because she brought it on herself because you could say she brought it on herself for reasons out of her control: i.e she might have been abused, born into poverty, have suffered a mental illness etc.

As long as she's an EX alcoholic then their backgrounds make no difference to their priority.
