Well you're best bet would be to ring/email them. I have some numbers for them, but they're all just called KCL in my phonebook so don't know which one is the main number, but they are:

I sent off one of these forms. The address should be on the letter you got with the form.

I keep missing calls from the occupational health dep. And keep forgetting to call them back. On the voicemail they left me they said something about booking an appointment for a health screening or something like that. Has anyone else been called by them??
