Default HC2 form - help with health costs
 
I managed to get one of these so I can get free prescriptions and dental treatment, but it also says I can get help with travel to receive NHS treatment. What exactly does this mean?

I've checked the HC5 form but it doesn't tell me much, it just says that I should ask for a refund when I get to the hospital.

I will be traveling by train to my hospital back at home. The cost of the train ticket is around twenty quid. Do they actually refund this back to me?

has anyone claimed a refund for travel?

Hello orangeboy! Yes, i have, as had to travel to London a couple of times to see a specialist. Just take your exemption form with you and all your travel tickets, if you take these to the reception, they will sort you out (or point you to the person dealing with expenses) and you will get your cash back there and then. I don't think they make that information clear either! take good care.

thanks for that.

I've been doing some research and it says they won't refund travel costs relating to primary care NHS dentistry, what does that mean, do you know?

I reckon it's to do with your local dentist.

The thing is, when I go to my hospital in a few weeks time, it's for a orthodontist appointment at the hospital, is this classed as primary care?

The regulations I read just don't say anything in plain English.

Hello orangeboy! i am sorry i did not get back to you sooner. From what i can understand (you're right, the info is not very clear!) due to you being referred by a dentist to an NHS dentist at the hospital, you should be covered to clain your travel expenses back.

You could have this confirmed though by ringing 0845 610 1112.

i attempted to fill in one of those forms and gave up cos it was far too confusing and they kept sending it back to me!
