On a flow chart, how do I show the following things:

User enters data (input: parallelogram)
Validate Info (process: rectangle (omit and include with below?))
Is information valid (decision: diamond)

is this correct? links to article?

and also: database query - does one include this in a flow chart?

User enters keyword

2.

Keyword searched in database

3.

keyword found (yes: go to #3; no: go to #1)

4.

list results on screen
