It depends on what one considers a failure. Although, I wouldn't consider AAD as a fail.
Anyway, here's my story.
Academically I was always in a middle ability set, however I was always considered as a person of lower ability by various teachers.
GCSE's were generally ok with a majority of C grades and one B grade. Hell, I was happy enough to pass GCSE Maths.
September 2010 - August 2011. So this was my first round of AS levels
I took History, Politics, French and Drama. Although, I thought I had been working hard I achieved four U grades for all of my subjects. That was probably the lowest point in my life.
August 30th 2011 - This was when I plucked up the courage to apply to a College. Mind you the College I applied to was a College that I thought was inferior.
"At least I was accepted" - That was my mentality at the time.
September 2011 - August 2012. I found myself at least rather happy with my new AS results.

September 7th 2012 - I tell my Asian side (Indian) about the fact that I am not going to university this year. The reaction wasn't as bad as I expected, considering that quite a few of my cousins were at Russell Group University's.
Skip forward to November 15th 2013. I've sent my UCAS off, anxiously awaiting replys. At this point I am expecting rejections.
November 17th 2012 - This happens to be my Birthday. I get an offer from the University of Essex for Modern History and Politics - BBB grade.
At this point I remember falling onto the floor in my bedroom crying with joy. - Yeah, that actually happened.
Eventually I got all five offers for History and Politics.
Skip to March 8th 2013 - January results time.

Politics Unit 3 Ideologies - B
English Literature paper resit - C (I got an E in that paper previously).
So here I am, a moderately middle/lower ability kid to failure to gaining pretty good AS and some A level
grades in my mind.
I've firmed Kent for History and Politics (need ABB)
Insurance is Oxford Brookes (need BBC)
For all those who have feel they've failed. Honestly, it could have been a whole hell worse. Whether you're facing hell or a festival just grab it by the testicle.

TLDR - Middle/Lower ability kid, did ok at GCSE. Failed AS Levels the first time with UUUU. Bounced back with BCDU - managed to get all five offers for History and Politics. Getting higher grades at A2, someone who is at least trying despite previous experiences.
