The ending of Halo Reach was pretty depressing.
Fallout 1/2 and a game that I think was called Warhawk? It's not the deaths but you always get a post-death message about how badly you screwed over the world by dying.
Since several people are saying Mordin, I'm adding Padok Wilks since he replaces Mordin if he died in ME2, but will die in the same way as Mordin does in ME3.

The obvious one would be Aeris in Final Fantasy 7, but I played through it for the first time this year and I certainly didn't cry (but my brother did as a kid). And to all you gamers that think FFVII needs to be replayed... please play it and see that you are as wrong as wrong can be!
