Can they? I would have thought any tampon would quickly become saturated with water, allowing blood to leak out into the pool.

No. The vagina acts like a seal so no water can get in. Wearing a tampon is not even necessary because of this.

No. It flows out relatively slowly. Plenty of time to go to the loo and put a tampon in or sanitary towel on before it leaks onto the outside of the swimming costume.
