Creative Writing/ Transfromation. A2 English Lit & Lang help.

I just started my second transformation, however I am having a writers block!! I was wondering, as a reader, what this means to you:

An unfamiliar, content sigh left his lips.

So here's my questions to you:

a)Would you read this as meaning it is a weird/unusual sigh
b)Would you read this as meaning it is uncommon of him being content (this is what I am trying to portray)
c) Is the comma needed

Thank you

a. No - for me, personally, when something is not familiar, doesn't mean it is necessarily out of the ordinary.
b. Yes.
c. Yes.

Thank you so much for answering my question. I just needed some clarification, because I am very indecisive
