You will unfortunately be limited as it's part of the assessment objectives, however don't worry too much, if you covered the other objectives well you can still get a good mark! :h: stay positive and try not to dwell on it too much as nothing can be done!

I don't think so, the only time I included another book in my exam was the second exam for OCR in which we compare an unseen extract to a text already studied. I'm sure the A Level exams include wider reading however.
I know that for AQA, 'wider reading' is not specifically what you have to do. The closest to that is AO4, which is connections to the tragic/romantic/whatever genre, where you might include comparisons to other books, but I've seen Band 5 answers that just talk securely about Aristotle's Poetics or general conventions within that genre. AO5 is perceptive analysis, and you might get that more easily if you had read a number of critical analyses, but you could always just come up with your own.

I don't know about any of the other boards.
