India and Pakistan are large enough and important enough to affect the rest of the world. E.g. Pakistan going against India would perhaps (I am guessing) cause a lot of economic distress in Asia, and even more anti-Muslim sentiment globally, especially between Hindus and Muslims.



Only if they care about Mutually Assured Destruction. It doesn't work if one side is ideologically crazed enough to suicide. But the main thing is, we can't neutralise Pakistan if they decide to do crazy stuff; their nukes limit our ability to protect ourselves. For example, Russia has been free to attack Ukraine, largely due to nukes, and also their geopolitical importance.

Islamic State is certainly crazier than Pakistan, but Pakistan is still crazy.

China is a stabilising influence on them; there is little risk of North Korea getting any more insane, since the insanity all stems from its ruler, rather than the people, and evidently Kim Jong Un isn't insane enough to risk nuclear war.
