If you don't use five choices now you can add them later on you don't make a new application to do it

As long as you haven't filled up all 5 choices you can add more later, for example i have some medic friends who haven't submitted their 5th choice yet since the deadline is in Jan. You use the same ucas account though but you may want to check about predicted grades with your school
