so i've done work experience and need a reference for it for my personal statement, do I ask for the manager's contact details or a letter? Also where to put the reference on the ucas formor personal statement or is it not needed until they ask for it?

You only have one reference for your UCAS application, and this should be an academic reference that comes from your school/college. You do not need a reference from work experience.
Your personal statement should show your passion and motivations for the course, and is definitely not the place to try to sneak in another reference
