URGENT - Study Swaps: Dissertation help

Hey Guys,
I'm in my final year at uni studying psychology and I'm struggling with my data collection. I'm doing a study on attitudes towards mental health. I'd appreciate it if you guys could help out and fill in the questionnaire for me. Any help would be appreciated.
