please give me advice about university work experience

hi all,
I have had a tough few months.. I have been having a combination of mental illnesses such as depression and social anxiety and lack of motivation. I finished my second year and I am due to start my third year in september.
My biggest problem is finding and completing compulsory 3 weeks of work before I start my third year. I really do not know where to apply and who would accept me at such short notice.
Please could I have advice and suggestions of who would accept me?

What is the criteria for the work experience?
What is its relevance?
How long have you had to organise it?
Do you have medical evidence of your anxiety and depression?
