UCAS application - supporting information autism help!!!

I have recently been diagnosed with ASD.
I want to disclose this on my ucas application but it tells me to add supporting information - "Your chosen universities or colleges will use this information to provide you with information on the support available".
What do I need to include?
What kind of things do they need to know?

Just put 'I have ASD' and with a few words about what help you need - ie. 'I need longer time in exams', 'I find crowded places difficult' etc etc.
You don't need to go into detail - whatever Uni you end up at will contact you to find out more details about the support you need before you start there.
