I'll do my best:

As mineral ions are transported into the roots by either active transport or simple diffusion, the water potential in the root cells become more negative than they were before. Therefore, water follows into the root by osmosis(down the water potential gradient as the root cells are now more negative).

This osmotic movement creates a high hydrostatic pressure at the root (due to the increasing volume of water now in the root). The hydrostatic pressure is lower further along the xylem so water can move through the xylem from the root, down the pressure gradient (from high to low hydrostatic pressure).

Remember - it is a theory. The evidence for it is the fact that you can cut the stem right through and xylem sap continues to flow out.
