I answered on a thread like this the other day, I said geriatrician, but apparently it's care of elderly now.

The true answer that it depends on your trust policy - some use elderly care consultant, some use care of the elderly consultant, some use pre-pathology co-ordinator. The generic term that is understood by everyone (nurses excluded) is geriatrician.

What bout Consultant Physician Acute Medicine with special interest in elderly care.
Dude, he said he was tight on characters. And the consultant may well not be a consultant in Acute Medicine (a specialty in its own right).
Geriatrician may not be the "official" term for PC sake, but in a situation like this it's probably still acceptable.
