The true answer that it depends on your trust policy - some use elderly care consultant, some use care of the elderly consultant, some use pre-pathology co-ordinator. The generic term that is understood by everyone (nurses excluded) is geriatrician.

Dude, he said he was tight on characters. And the consultant may well not be a consultant in Acute Medicine (a specialty in its own right).

Geriatrician may not be the "official" term for PC sake, but in a situation like this it's probably still acceptable.
