Applying for my first full time job..., this might sound like a really stupid question, but just unsure of when and how formal to be...

I have got an apprenticeship job description saying email .... for an application form.

Do I just send an email saying: please send an application form to...

OR

Dear Sir/Madam,
I am interesting in the IT apprenticeship, please send an application form to:...

The formal one but you should write 'interested' rather than 'interesting'.

It states to ask for an application form on the job description, maybe that would be more detailed than a CV.
